Penetration Testing and Continuous Offensive Assurance

Why an annual penetration test leaves a gap, and how RXG Labs combines human testing with Pentera, SafeBreach and Synack under one accountable, governed programme.

This article presents The Continuous Offensive Assurance Value Proposition (Risk X Group, October 2026): how the RXG Labs penetration testing practice is complemented by Pentera, SafeBreach and Synack to turn testing from an annual event into a continuous, governed programme. The full document is available as a PDF at the end of this page.

The gap between annual tests and a changing attack surface

Many organisations still buy assurance as an annual penetration test: a skilled team, a fixed scope, a report, and then eleven months of drift. Attackers do not work on that calendar. New assets go live weekly, cloud identities sprawl, controls are reconfigured, and new exploits appear within days of disclosure.

The result is a gap between what was tested and what is exposed, with less than a third of attack surfaces tested at all.

Boards and regulators are increasingly no longer asking “did you pentest?” The real question is more often “can you prove your defences work against the attacks that matter to you?”

RXG’s model: people, automation and governance

RXG’s view is that modern security is collaborative: intelligent automation, skilled practitioners and strong governance working as one model. RXG Test applies that model to offensive security, turning penetration testing from an event into a programme.

One accountable, CREST-accredited partner combines four complementary engines, each chosen for what it does best:

  1. RXG Labs brings human depth: certified testers who think like adversaries, chain findings and explain business impact.
  2. Pentera brings automated breadth and frequency: safe, repeatable attack-path validation across internal, external and cloud estates.
  3. SafeBreach brings control assurance: proof that EDR, SIEM, email, web and network controls actually prevent or detect real attacker behaviour.
  4. Synack brings elastic scale: a vetted global researcher community plus agentic AI for applications, APIs and fast-moving attack surfaces.

Technology finds and repeats; RXG experts interpret, prioritise and prove. The client gets one scope, one risk language and one remediation roadmap, instead of four tools and four reports.

Governed automation, not unchecked AI

Pentera, SafeBreach and Synack’s Sara all bring AI and automation into testing. RXG wraps them in the same governance it applies to its own work:

  • Clear oversight of what is tested, when and how.
  • Human validation of every critical finding before it reaches the client.
  • Reproducible, evidence-based results that stand up to audit.
  • Consistent quality assurance and risk rating across every engine.

AI should strengthen trust in security testing, not erode it. That principle is what separates an RXG programme from a stack of tool subscriptions.

RXG Labs: human testing and accountability

RXG Labs is the accountable core of every engagement. It is a CREST Pentest accredited member (7708705-3), and its permanently employed testers hold OSCP, OSEP, OSED, CRTP, PNPT, CISSP, CISM and related certifications. Testing follows a customised framework aligned to NIST SP 800-115, OSSTMM and OWASP.

RXG Labs delivers what automation cannot:

  • Web, mobile, API and code-assisted application testing.
  • Internal and external infrastructure testing.
  • Wireless testing.
  • PCI DSS testing built for expedited QSA acceptance.
  • External attack surface mapping.
  • Objective-driven red teaming.

Every report carries OWASP risk ratings, attack-chain graphs, finding walkthroughs and Excel exports for tracking. Remediation retesting of up to one day is included, and prepaid testing bucket days can be drawn down for retests or new scope, so the engagement ends with residual risk, not just a list of findings.

Pentera: automated validation between human tests

Pentera runs real, safe attack techniques against the live environment and validates which exposures are actually exploitable. Its platform covers:

  • Pentera Core: internal networks.
  • Pentera Surface: the external perimeter.
  • Pentera Cloud: cloud identity and hybrid environments.
  • Pentera Resolve: orchestrating remediation.

In the RXG programme, Pentera turns the annual infrastructure test into a monthly or on-demand cycle. RXG Labs runs and tunes the campaigns, validates the attack paths that matter, and folds the results into the client’s risk register. When a new critical CVE lands, the client can confirm exposure within days rather than waiting for the next test window.

SafeBreach: proving the controls work

A pentest shows how an attacker gets in. SafeBreach shows whether the defences would stop or see them. Its Exposure Validation Platform pairs breach and attack simulation (SafeBreach Validate, with a playbook of 30,000+ attack methods) with post-breach attack path validation (SafeBreach Propagate). Every simulation is scored as prevented, detected or missed.

RXG uses SafeBreach to measure control efficacy against the threats relevant to each client: ransomware families, sector-specific actors and newly published advisories. The output answers a board-level question with evidence: what percentage of relevant attacks our controls stop today, and whether that number is improving. It also gives the SOC concrete detection gaps to close, which aligns directly with RXG’s Secure X services.

Synack: elastic scale for applications and fast-moving surfaces

Synack delivers penetration testing as a service through the vetted Synack Red Team and Sara, its agentic AI red agent. Sara expands coverage and triages scanner output to confirm which findings are truly exploitable; human researchers prove impact across web, API, cloud, mobile and AI/LLM targets. All testing runs through a controlled, fully logged gateway.

Within the RXG programme, Synack absorbs peaks the in-house team should not: large application portfolios, continuous testing of release cycles, and specialist skills on demand. RXG Labs remains the client’s single point of contact, owning scoping, quality assurance, risk rating and remediation verification, so scale never comes at the cost of consistency.

How the four engines fit together

Each engine answers a different assurance question; RXG Labs connects the answers into one risk picture.

EngineQuestion it answersTypical cadenceBest fit
RXG LabsHow would a skilled adversary reach our crown jewels, and what is the business impact?Annual or per release; red team per objectiveCompliance-grade tests (PCI DSS), complex apps, red teaming, code review
PenteraWhich exposures in our network and cloud are exploitable right now?Monthly or on demandLarge infrastructure estates, post-change validation, new-CVE checks
SafeBreachDo our controls prevent or detect the attacks that matter?ContinuousSOC tuning, control ROI evidence, board reporting
SynackAre our applications and changing perimeter secure at scale?Continuous or point-in-timeLarge app/API portfolios, DevOps release cycles, specialist skills

A typical client journey

  1. RXG Labs baseline test.
  2. Pentera and SafeBreach for continuous validation.
  3. Synack where application volume or release velocity outpaces a fixed team.
  4. One remediation roadmap, retested to closure.

Findings from every engine flow into one RXG-owned remediation roadmap and are retested to closure.

Why RXG: what the client gains

  • Assurance that keeps pace. Exposure is validated continuously, not once a year, so the risk picture reflects today’s estate.
  • Evidence, not opinion. Exploitability is proven and control efficacy is measured, giving boards, auditors and regulators defensible numbers.
  • Less noise, faster fixes. Automation and AI triage strip out non-exploitable findings; RXG experts prioritise what genuinely reduces risk.
  • One accountable partner, nationally. A CREST-accredited, South African team owns scope, quality, risk rating and retesting across all four engines, with offices in Cape Town, Johannesburg and Durban and Rand-based pricing.
  • A path from test to resilience. Results connect across Risk X Group: Risk X governance and compliance, Wolfpack threat intelligence and cyber risk advisory, Secure X SOC and managed detection and response, and incident response. Findings become improved controls, detections and board reporting, not shelf reports.

RXG enables organisations to stop asking “when was our last pentest?” and start answering “how secure are we today?”
